What do I need to bring to my consultation appointment?

• A Valid Colorado Photo ID or Driver's License
• Medical Documentation supporting your condition (CLICK HERE to find out how to obtain your medical records)
• Doctor recommendation visit $200.00 cash or credit card (credit card fee applied)
